To Apply for a Dog
1) Please fill out
the application
answering all the questions listed. We do suggest that the
applicant be a minimum of 12 years old. Please send us a
video of yourself on a normal day at home, work/school and at
leisure.
2) When your application has
been received and reviewed a personal interview will be conducted
here at our facility in Frenchtown, NJ or at our training space
in Lebanon, NJ. You will be interviewed by a Dogs In Service
instructor or worker. This gives us the opportunity to go over
your application in more detail and gives you the opportunity to
ask any questions you may have.
3) Your doctor, physical
therapist and references will be contacted.
4) You will be notified if you
have been accepted as a future client by Dogs in Service and, if
accepted, your name will be put on our waiting list for an
assistance dog. You will be told at your interview the
approximate wait time for a dog.
5) Once a dog has been selected
you will receive a tentative date for training and pictures of
the dog selected.
6) A recipient of a service dog
must commute daily for 3 weeks (Monday through Friday) to
Frenchtown or Lebanon Plaza for training. Training is scheduled
from 10am to 3pm. Any transportation (long distance or local),
hotel/motel lodging, meals, personal care attendant (if needed)
are the financial responsibility of the client.
7) Each client must successfully
complete the training schedule in order to receive his/her
assistance dog.
8) A graduation ceremony
completes the training process. Friends and relatives may attend
and it is open to the public.
9) The cost to raise/train a
service dog is between $12,000 and $24,000 dollars. The cost from
Dogs in Service is between $1000.00 and $3000.00 plus a $300.00
equipment fee. This fee is due, in total, when training is
scheduled. If it is a hardship to pay this dog and
equipment fee, time payments may be arranged. If a recipient is
able and willing to pay more than the required minimum, he/she
may do so.
